Dual Diagnosis Services
Heritage Oaks takes a multidisciplinary approach to treating individuals with co-occurring psychiatric disorders and chemical dependency. Dual diagnosis services include inpatient care, partial hospitalization and outpatient programs. We are contracted with most insurance companies.
Inpatient Care
Inpatient care provides a structured and supportive environment in which patients learn to modify behaviors that have made life difficult to manage.
An interdisciplinary team of professionals — led by a psychiatrist and including a nursing staff, a clinical therapist and a case manager — works with the patient to identify problem areas and develop an appropriate treatment plan.
Some patients stay for only a few days to resolve a crisis; others may stay longer for more intensive treatment. Individual attention is an integral part of our strong commitment to quality care.
Our dual diagnosis program follows the 12-step disease model of treatment, which has provided millions of people with a life of recovery.
Our goal is to alleviate the immediate co-occurring acute psychiatric and chemical dependency symptoms that necessitate inpatient care, while preparing the patient for the next, lower level of care and appropriate outpatient services.
Upon Discharge
Outpatient care provides the next important step in continuing the recovery process. Successful recovery and transition back into productive home, work and family environments depends greatly on the quality of care patients receive following inpatient treatment. Our comprehensive services include a Partial Hospitalization Program (PHP), Intensive Outpatient Program (IOP) and a Chemical Dependency Intensive Outpatient Program (CD-IOP).
Heritage Oaks Hospital offers a dual diagnosis track within both the PHP and the IOP at three convenient locations. Dual diagnosis-track patients can begin to integrate useful tools they learn in educational and therapeutic groups at home and in their work environments to strengthen and build on their own recovery process. Certified CADC, therapists and dual diagnosis counselors provide help and support on the road to recovery and a better quality of life.
Heritage Oaks also offers a Chemical Dependency Intensive Outpatient Program (CD-IOP) for patients who have completed inpatient rehabilitation, detox or residential rehabilitation. CD-IOP provides the educational and treatment components while allowing patients to apply their newly acquired skills within “real world” environments. Classic 12-step models as well as alternative concepts in recovery are used. We can arrange for medical and psychiatric consultation, psychopharmacological consultation, medication management and crisis services.
