The Admissions Process
The admission process begins with a thorough assessment completed by a licensed, trained Assessment and Referral clinician. Assessments can be completed by phone or on-site.
Once the assessment is complete, the clinician reviews the criteria with the attending physician, and the appropriate level of care is determined. The clinician will also contact the insurance provider, if applicable. Heritage Oaks Hospital is an approved provider for most insurance companies and managed care systems, including Medicare and Medi-Cal (for individuals over 65 or under 21). Alternative methods of payment will be discussed if insurance is not available.
Once the clinical need and appropriate level of care are established, the review process is completed, with admissions often taking place on the same day as the assessment.
Upon admission, nursing staff will complete a clinical assessment and provide an orientation to the unit. The patient is then assigned to a psychiatrist on staff.
A medical doctor will complete a history and physical exam with the patient and will follow his/her care throughout the hospital stay.

For referrals, please call either (916) 830-2223 for inpatient referrals or (916) 914-1388 for outpatient referrals.
Confidentiality
To protect our patients’ confidentiality, they are each given a patient confidentiality number upon admission. This number acts as a code that will allow the staff to acknowledge that the patient is at our hospital.
If a patient wants to have detailed information about his/her hospital stay shared with anyone, the patient must sign a Release of Information form to facilitate the sharing of any such information.

- Three pairs of pants
- Three shirts / blouses
- Three to four pairs of underwear
- Three bras (no underwire)
- One pair of shoes (no strings) / sandals / flip-flops
- Three pairs of socks
- Two sets of night clothes (pajamas)
- Cosmetics and toiletries in plastic containers
- Two sweaters or sweatshirts (no strings)
- Optional: Shampoo, toothpaste, mouthwash (no alcohol content), powder, lotion
What not to Bring
- Strings
- Spaghetti straps
- Revealing clothes
- Sharp ornaments
- Short shorts
